2 Ways the “Paper Clip Dentist” Scandal Affects Dentist-Patient Relationships

Wow. If people weren’t already afraid of the dentist, they’re going to have a tough time now. Or rather, practicing and future dentists will have a harder time convincing patients they’re trustworthy thanks to the latest dental lawsuit making waves in the media.
On Monday, Dr. Michael Clair of Massachusetts was sentenced to one year in jail because he used paper clips instead of stainless-steel posts for root canals and then fraudulently billed Medicaid the full price. One of the patients involved in this particular case, Joshua Almeida, suffered toothaches and eventually lost his tooth from the botched root canal.
What can we take away from all this?
I think there are a number of lessons to be learned from the “Paper Clip Dentist” scandal, but two things in particular stuck out to me.
Safety Over Cutting Corners
The paper clip post and core method for root canals may be familiar to some dental students. It’s not unheard of for a root canal project in a third year Restoration course to use a paper clip for the temporary core of a provisional crown. Even one of the most frequently cited textbooks on the subject, Fundamentals of Fixed Prosthodontics by Herb Schillingburg, DDS, includes this less than optimal material where you would expect a stainless-steel post to be used. However, it’s important to note that it is considered only for temporary use.
Clair acknowledges he used the common office supply because it was cheaper than the $50 a piece stainless-steel posts. He might have gotten away with it for the temporaries, but he kept them in permanently leaving unaware patients with structurally unsound teeth and vulnerable to infection.
It’s no secret that running a dental practice is expensive. Of course you’ll want to look for a way to cut costs when you’re dealing with massive student loan debt from dental school, keeping equipment up-to-date, and the everyday business expenses of rent, utilities, and payroll. But safety should always trump cutting corners financially. Granted, Clair may be suffering from some sort of mental issue that impairs judgment, but I believe his extreme case serves as a reminder not to cross ethical boundaries when under financial stress.
Easing Dental Fear
An estimated 75% of Americans are fearful of the dentist with up to 10% of those avoiding dental appointments all together. Much of the anxiety associated with dental work is a result of previous bad experiences whether it was a painful procedure or a poor relationship with a dentist. However, news stories such as this most recent lawsuit can horrify patients indirectly.
There’s a good chance that you’ll be faced with even more challenging dental patients than before. After all, root canals are already one of the most feared procedures and now they’ll shutter every time they reach for a paper clip at work. It may be time to boost your chair side manner in regards to this heightened anxiety.
Communication is everything. Keep your ears open and the conversation going with each patient. Some may not want to know what’s happening at all, but many relax when a full explanation of the procedure is given. Perhaps now would be a good time to start showing them the materials you’ll be using so they know you don’t shop at Office Depot for dental supplies.

Providing quality care and squashing fears make for a successful dentist. If you win them over while they’re in your chair, they’ll return frequently and brag about you in between appointments. As a bonus, you’ll get to feel all warm and fuzzy inside.

10 FAFSA Tips for Health Students

Asany student can tell you, paying for school can be the most difficult part ofall. But luckily, there are literallymillions of students each year who have the same problem. No matter if you are studying dentistry,radiology, or becoming a brain surgeon, there are financial aid options foryou. To prove it, we have included justten tips every health student should know.

1. FAFSA– The Free Application for Federal Student Aid should be the first item anystudent attending any college should take on. A copy can easily be obtained online, at your high school, or at thecollege you are considering attending.

2. CorrectFAFSA – The rules to financial aid are always changing and so are the FAFSAforms. Before you fill yours out, makesure that it is for the current year, as last year's form may not apply.

3. Earlyis better – Because the FAFSA is such a lengthy form, be sure and get startedas early as possible as it will ask for all your information, your income, yourparent's income, and much more.

4. Doit online – If you have an internet enabled device at home, use www.fafsa.ed.govto fill out the FAFSA. It will take allthe usual hassles away from filling out paper forms and even has tips as youfill it out.

5. Dependentor not – If you are still living with your parents, chances are you might be adependent. However, those who don't orwho do live with their parents and pay for their own school might be classifiedas independents. Know which you are andwhich you should be to ensure the best results.

6. It'sall free – If you have questions about filling out the FAFSA or any financialaid form, don't pay someone to help you. The counselors at your high school or future college are paid to knowthe answers and can help you at no charge.

7. Deadlines– As with most things, there are deadlines for the FAFSA. There is an early, on-time, and latedeadline. Getting your FAFSA in by theearly one has many advantages including the option to reapply if your firstapplication is wrong and the option of seeing how much financial aid youqualify for, which can help you decide between colleges.

8. Noblanks – Even though an item on the FAFSA may not apply to you, it is importantnot to leave any line blank. If you arepositive the item does not apply to you, put in a “0.” If unsure, ask a counselor.

9. Nolying – It may be tempting to lie about something like income to get betterfinancial aid results, but FAFSA will use you and your parent's income taxinformation to confirm your answers, so be sure and do it right.

10. Repeat– Most students plan to attend school for at least two years, and the FAFSA andfinancial aid process should be repeated for every year of school. This is doubly true for students whosefinancial circumstances change, as the amount of financial aid can change too.

Bonus! BeyondFAFSA – FAFSA is the standard for financial aid, but many schools have theirown forms as well. These are also to befilled out for every school you plan on applying to. The best part is if you fill out and turneverything early enough, you will know how much the school will cost out ofpocket.

Fabrication of Removable Partial Dentures

You may recall my post some time ago about Removable Partial Denture Design. If not you can find it here. Now that the design is complete, it's time to fabricate the denture.

Here's the RPD Design

This, according to the Kennedy classification is a Class II modification II arch.

After an alginate impression is made of the arch from the dentoform, it is then poured up in plaster.
Though not very visible, the design/outline of the RPD is made on the cast
Wax is placed to block out undercuts

A record base is then made of triad material following the outline that was made on the cast
It is then cured, which makes it hard

Wax is then added to the edentulous spaces until contact is made with the lower arch

Alu-wax is added to articulate the upper arch with the lower arch

Endodontics: Access Opening

We start endodontics this semester. So far we've been learning how to open a tooth and access the root canal. We were also introduced to some of the basic instruments of the trade as well as the type of burs used to access the canal.

Here are some of the teeth that I've accessed so far. Some are missing since I turned them in for grading.

Steps to follow for accessing the canal:

High speed hand piece with EAD4 bur to enter through enamel and dentin;
followed by the use of a slow speed hand piece with the EARC4 bur to access the root canal and plane the canal walls.
At this point a broach can be used to remove pulpal tissue.
Using high speed han piece again the EARC4 or even the XGATE can be used to shape the access opening with guidelines for the particular tooth you are working on, eg: triangular outline for a maxillary central incisor or Oval outline for a canine.

ASDOH's The Dental Anomalies

I got introduced the other day to Dentistry's First Music Band: The Dental Anomalies.
The Dental Anomalies, Nima Afsari and Mark Anthony Williams II, are second year dental students at The Arizona School of Dentistry and Oral Health (ASDOH). They started blending their passion for dentistry and music when they first met and created a whole album which illustrates what it is like to be a dental student. The Dental Anomalies enjoy melodic sounds and try to incorporate many genres into their music, such as rock, rap, pop and jazz. With the release of their first album, the future of dentistry and music looks promising.

Removable Prosthodontics: Complete Dentures

Earlier, I made several posts about complete dentures and setting teeth. Since then, I've done a couple more and of course, with practice everything gets better.

1st Set-Up:

This was my very first set-up. Of course at the time I thought this was great
but looking back...yikes! You can clearly see that the left side is slightly higher
than the right, and the wax job is nothing to call home about.

Upper Arch

Looks a bit like it's leaning to the left an the 2nd molars are sticking out
too far bucally.

Lower Arch:

Lots of faults: Right canine is making the distal turn, lingual gingiva on
the right too thin, left 1st premolar sticking too far bucally, etc etc

2nd Set -Up

Looks much better than the first, neater, more polished...
However, left posteriors sticking too far bucally.

Upper Arch

Some areas needed wax re-enforcement. 2nd molars might have
been placed too far bucally.

Lower Arch

Looks okay to me

3rd Set-Up

Good, better, best right? In terms of occlusion, presentation, etc this one
turned out best.

20 Questions with Dr. M. Rogers DDS (Ortho edition)

While perusing the Articles and Interviews section of the Student Doctor Network, I came across an interview with Orthodontist Dr. Michael Rogers who describes the field of Orthodontics and pinpoints the reasons he chose the field and why he thinks the field is the best specialty.
Some points mentioned are note worthy if you are thinking of specializing in Orthodontocs.

The article/interview:

Dr. Michael B. Rogers owns a private orthodontics practice with his colleague Dr. Andrews in Augusta, Georgia. He graduated from Emory University School of Dentistry before serving as a Captain and general dentist in the U.S. Army Dental Corps. Later, he returned to the Medical College of Georgia to specialize in orthodontics, earning his Certificate in Orthodontics from the Georgia Health Sciences University (Medical College of Georgia) School of Dentistry.

Dr. Rogers is currently on the orthodontic faculty at the Georgia Health Sciences University and is a Diplomate of the American Board of Orthodontics. He has received numerous professional recognitions, including those from Georgia Association of Orthodontists (Exemplary Service Award, 1991 and 2006), Oren A. Oliver Southern Association of Orthodontists (Distinguished Service Award, 2002), and Emory University School of Dentistry (Meritorious Service Award, 2009).

Dr. Rogers has been active in several professional organizations, including the American Dental Association, Georgia Dental Association, American Association of Orthodontists, Southern Association of Orthodontists, Georgia Association of Orthodontists, Medical College of Georgia Orthodontic Alumni Association, Georgia Health Sciences University School of Dentistry Steering Committee, College of Diplomates of the American Board of Orthodontics, Eastern District Dental Society, and Georgia Academy of Dental Practice. He has been published in the Journal of Clinical Orthodontics, Orthodontic Products, and the American Journal of Orthodontics.

When did you first decide to become an orthodontist? Why?
When I was a sophomore in dental school, we had an orthodontic lab and I enjoyed the challenge of bending the wires. In addition, I began to realize how orthodontics can improve a person’s self esteem by improving their appearance. It can make a difference in the job offers they receive, and even their selection of a spouse.

How/why did you choose the school you attended?
I lived in Augusta and selected Emory University (in Atlanta). Its dental school had an excellent reputation. Emory was also the only dental school in Georgia, South Carolina, or Florida at that time. My orthodontic education was at the Georgia Health Sciences University, formerly the Medical College of Georgia.

What surprised you the most about your studies?
I was surprised in dental school by the amount of lab work and dexterity that were involved. Being “good with your hands” is very important for a successful dental career.

If you had it to do all over again, would you still become an orthodontist? (Why or why not? What would you have done instead?)
Yes, if I had it to do all over again, I would still become an orthodontist. Orthodontics has been a rewarding career from all aspects. It has allowed me to influence many lives through improving their smiles.

Has being an orthodontist met your expectations? Why?

Yes, being an orthodontist has more than met my expectations, because it has allowed me to influence many lives by improving their smiles. In orthodontics there are no failures since everyone experiences some improvement. It is very rewarding to see the big smile when the braces are removed, and in many instances the improvement in self esteem.

What do you like most about being an orthodontist?
As an orthodontist, I most enjoy dealing with people and the many variations in orthodontic cases. Some cases involve jaw surgery while others require functional appliances (growth appliances). In addition, I enjoy seeing the “big happy smile” at the end of treatment.

What do you like least about being an orthodontist?
Like any business that depends on appointment keeping, I am sometimes frustrated with the way the schedule can become hectic if patients are running late. On occasion, the schedule is delayed by patients whose appliances are loose and in need of repair. In addition, some patients will not brush their teeth or cooperate in other ways, which can lead to a lot of frustration. Patients can compromise the results of their treatment if they do not keep up with home hygiene, diet recommendations and professional prophylaxis.

Specialize or Not?

Earlier you might have seen me mention my interest in specialty, particularly Orthodontics or Oral & Maxillofacial Surgery (OMFS). While I find the OMFS procedures very interesting, whilst doing further research on the field, I have decided that the lifestyle that profession affords is definitely not for me.

So I'm back at the drawing board.

I'm still very interested in Orthodontics but since my debut in the Sim lab at the start of my 2nd year I think General Dentistry might be great after all (keep in mind I have not started seeing patients yet). I find myself asking this question a lot lately: Should I specialize or not? The answer to that question is not yet apparent but in the meantime I have decided to continue doing my research on the specialties and perhaps spend some time shadowing or assisting different specialists in hopes of arriving to an answer to my question.

When I figure it out you'll be the first to know.

Studying for the NBDE I: The beginning

I recently started studying for the NBDE Part 1 in hopes of sitting it some time in the early Spring of 2012. I will try to give a few updates as I go through the process, and some handy dandy tips once it's all over.
As you may know, the NBDE is comprised of the following sections:

Human Anatomy and Histology
Biochemistry and Physiology
Microbiolgy and Pathology
Dental Anatomy and Occlusion

I have decided to use the following materials as my guide as well as a few other resources from the stack of text books that have been helpful throughout the past year and a half.

1. Dental Decks Part 1 2012-2013
2. First Aid for the NBDE Part 1
3. Kaplan NBDE Part 1 Lecture Notes

Plan of attack:

I will be using the materials above simultaneously as I study each section and I will be continuously testing myself on each section completed.

First thing: Take a practice test to see where I am at this point. This is simply to answer the question: If I were to take the exam today how would I score?
I will then cover Dental Anatomy and Occlusion because I think that's relatively easy and I should be able to cover it fairly quickly.
I will then attack Microbiology and Physiology because those are most fresh and hence should also go by fairly quickly.
Then I'll move on to Anatomy and Histology, then Biochemistry an Physiology where I'll spend the bulk of my time.
These will be done while continuously taking section tests to see my mastery of each section. when I have completed reviewing and/ or studying each section then I will begin taking full tests until test day (which I have yet to set)

This is the plan for now, things may change so stay tuned!

About The NBDE Part I

Now that final exams are over, it's time to ~~go on my well needed Christmas vacation!~~ study for the NBDE Part I (womp).

Different schools have different curricula which affects when we take the NBDE I. However, most dental students are fully ready and equipped to take this exam after their 2nd year and before they enter the clinic. As for me, the aim is to take it in the Spring of the 2nd year.

What is it?: The National Board Dental Examination, as stated by the ADA is given to assist state boards in determining qualifications of dentists who seek licensure to practice dentistry. It is taken to assess the competency of the applicant in the basic sciences and pre-clinical sciences.

How to register for the exam: The process of registering for the NBDE is similar to that of registering for the DAT (assuming you are a dental student and have already been through this process). After going to the NBDE section under the Education & Careers tab on the ADA website (here) you then register for a DENTPIN. After receiving your DENTPIN you can proceed to pay for and register for the exam.

Fees: The fee for the NBDE I is $300. It is non-refundable and includes sending a copy of the score report to the Dean of your Dental School. Any additional score reports required is an additional $25.

Scheduling: After you have registered for the exam, you will have to schedule a date and time to take the exam at a Prometric center via the prometric website (here).

Structure: The NBDE I is an 8hr exam consisting of 400 multiple choice questions based on the basic sciences, with topics including:

Human Anatomy and Histology
Biochemistry and Physiology
Microbiolgy and Pathology
Dental Anatomy and Occlusion

and administered in the following order:

Optional Tutorial .............................................................................. 15 mins

Discipline based, multiple choice test items

with 3-5 testlets (approx. 200 items)................................................. 3.5 hrs

Optional Break ................................................................................ 1 hr

I may not need to plug this, but make sure when taking the NBDE exam that you are ready so that you avoid a ~~bad score~~ Fail (F) and a re-take.

The Specs:

Depth of Field - The range of distance that is in focus without moving your head. This deteremines how much you lean in or lean back while wearing your loupe, and still maintaining focus of the entire viewing field. A large depth of field is ergonomically beneficial since you would not need to move your head or back much to work effectively.

Field of View/Field Width - This is the size of the operating site seen through the loupes. Having a wide field of view makes it easier and quicker to adjust from the loupes to the naked eye. It also causes less eye fatigue. * the higher the magnifying power, the smaller the field. * the longer the working distance, the larger the field of view.

Working Distance/Angle- The distance between your eyes and the patients oral cavity when sitting in a normal ergonomic position. This varies by each person's height, body type, or how you sit while with patients. Because the correct working angle allows you to sit in the correct ergonomic position it prevents neck and back pain.

Flip-Up versus TTL type - Each has their specific Pros and Cons and depends on your preference. Refer to Dental Loupes part I for pros and cons.

Resolution - The capability to view small structures and the clarity at which they are seen. This is established by the optical design and the use of precision lenses.

Magnification - This is the size of the object seen through the loupes (not clarity).

There is no accepted standard for how magnifying power is defined for surgical loupes. Different definitions give results that differ by as much as 15-percent. Because of this, the quoted magnifying power is really only a rough guide to the actual magnification of a loupe.

Surgitel is pretty good in my opinion, it has a wide variety of loupes with magnifications from 2.5x to 8x. The Oakley frames are a popular option but I personally like the Ultralight TTL. It has 2.5x and 3x capability, expanded field sizes and long depth of fields. They are also uniquely stylish!

Orascoptic is one of the more popular choices with magnifications of 2.0x to 5.9x. I personally really like orascoptic HiRes2 because they are light weight, has high resolution with a wide and deep field, plus they are stylish and comfortable.

Sheer Vision loupes are another set of loupes that I think are really good. They come in a variety of types from flip ups to TTLs with a few frame options. They have magnifications from 2.5x to 3.5x. They sport an expanded field of view, exceptional depth of field, sharp ede to edge clarity and are multi coated for anti glare, anti scratch and anti fog resistance.

Design for Vision loupes has magnifications from 2.5x to 4.5x and comes with designer and sport frames like Nike as well as other types.

Heine................................................................................www.heine.com

Heine loupes has magnifications of 2x to 2.5x. According to Dentistry Today, Heine loupes are unique in their ability to relieve stress of the practitioner. It features a 3 point articulation system that allows a higher angle of declination than TTLs. The i-view S frame allows for complete adjustment of the angle of declination from a 0 to 45 degree angle.

Loupes are an expensive but worthwhile investment. It is important that you do your research before making a decision and a commitment.

NOTE: If you contact a representative from any loupe brand, they are usually happy to have you borrow a pair for a couple weeks to try them out (for FREE). Try as many as you want to see which one's best for you.

In The Loupe: Dental Loupes I

Yes, I am referring to the glasses/goggle looking magnifier of all things small and intraoral. I've been a bit out of the ~~loupe~~ loop when it comes to loupes. There seems to be a lack of information available to help dental students decide on if or what type of loupe to get. Nevertheless, I've decided that I do want loupes, the type or brand of loupe, well, that's another story. There are a lot of specifications to take into consideration and a world of gimmicks to avoid.
So let's talk a little bit about Loupes.

Why Get Them?
Magnification of small intraoral structures, and most importantly ergonomics. One of the major concerns for dentists and dental hygienists is work related injuries. Long hours of awkward sitting, neck posture and eye strain from working in such tiny quarters can really take its toll leading to serious consequences which can be threatening to your career. Because of the magnifying capabilities of loupes, they offer:

Reduced eye, neck and back fatigue

When To Get Them:
In my opinion, students should get them when they start working in the simulation lab for ergonomic reasons.

Types of Loupes:
Flip-Up - These have the magnification barrels suspended over the glasses by a pivoting arm attached to the nospiece of the glasses.

Pros:

Costs a lot less than TTL

Can easily flip them up when not in use (example: when talking to a patient)

Easy to add a prescription frame

adjustable angle of inclination

Cons:

Can incorrectly adjust the angle of inclination leading to eye and musculoskeletal problems

Weigh significantly more than TTLs which can put additional strain on neck and back

They have a narrower field of view because they are mounted farther away from the eyes

Through The lens (TTL) - These have the magnifiation barrels directly on the lens of the glasses.

Pros:

Weighs less than flip ups

Provides wider field of view

Custom fitted for you personally so no adjustment needed

Cons:

Costs more than flip-ups

If you have a prescription for your lens and your prescription changes,

you will have to send it to the manufacturer to be adjusted ($$)

You have to take loupes off or look up through the lens while talking to patients.

Removable Partial Denture Design

Removable Partial Denture - this is sometimes used for patients that are missing some, not all of their teeth (partial), where a fixed prostheses is contraindicated, where cost doesn't allow or just a preference of the patient.

There are different ways to classify an edentulous mouth. The basics are:

Class I (bilateral free ended partially edentulous)
Class II (unilateral free ended partially edentulous)
Class III (unilateral bounded partially edentulous)
Class IV (bilateral bounded anterior partially edentulous), etc

As future dentists it is important that we pay attention to the trend that's observed in this video. America is facing some tough challenges at present. It is of paramount importance that we understand how the need for dental care and dental literacy fits into the scheme of everyday life. The dental profession, while a moving target, must continue to maintain its prominence in the life of everyday citizens; however, as history has shown, the prominence of this profession is as good as our ability to convey its importance. Failure to do so can only seek to exacerbate some of the problems as observed in this piece, which doesn't bode well for our society. Fixed Prosthodonics: Crown Preps

Now that Operative Dentistry is over, It's time to start Fixed Prosthodontics. We started with crown preps and here is my first:

Proximal & Height of Contour Reduction

Buccal & Lingual Reduction - 0.8-1.5mm

- In the first plane I made the axial walls parallel

then tapered in the second plane - approx. 3-6 degrees

-Ensured maxillary lingual cusps were tapered towards

the central fossae of the opposing mandibular tooth

- Kept original form, outline and orientation of the tooth

Occlusal reduction- 1.5 - 2mm

- Reduced occlusal surface following the outline

of the cusps and keeping the central groove

Oral Myiasis

Oral myiasis is a rare disease in humans associated with poor oral hygiene, suppurative oral lesions, alcoholism and senility, etc. It's caused by fly larvae! Though sterilized fly larvae is sometimes used to treat stubborn non-healing ulcers (by eating the necrotizing tissue while leaving viable tissue behind), this is not the case here:

A psychiatric (schizophrenic) patient presented to The University of The West Indies Dental School with this problem:

Yes! those are maggots in his palate

Other cases:

Treatment includes surgical exposure of the affected area, physical removal of the maggots and cleansing with either topical or systemic medications and, wound debridement.

Suture him up, give him some antibiotics and he's as good as new! or close.

Dentist License Revoked for Administering Botox

Helaine Smith, a well known Massachusetts cosmetic dentist, lecturer and author who has been practicing for ~ 20yrs, was disciplined and got her license revoked for unauthorized use and administration of Botox (Botulinum toxin).

Apparently, in an article written in The Boston Globe about the increased demand for Botox treatments, she told the paper she has been using them for quite some time. This piece of information caught the attention of the state's Board of Registration of Dentistry and they later launched an investigation. The investigation revealed the unauthorized administration of Botox to patients as well as other violations including; failure to conduct weekly tests for sterilization, failure to correctly store dental instruments, administering sedation without a permit, etc.
According to the law, general dentists are not allowed to administer Botox to patients, this can only be done by a certified Oral and Maxillofacial Surgeon who has been trained in the use of Botox and dermal fillers and can only be used for the treatment of disease, disfigurement and dysfuntion.

The punishment is pretty harsh but it reminds us that we should keep abreast of state and federal law and regulatory changes as well good practice management.

Removable Prosthodontics: Mounting Casts

Last time I posted about Removable Prosthodontics I left off at Occlusal (wax) Rim:

Next step is to mount the maxillary cast.

After the articulator is put together and the condyles, incisal table etc is set at the right angle, the bite fork is wrapped in wax and seats the cast (with record base and wax rim). Mounting ring is screwed in place then it's time to mix, place and shape plaster, giving:

and mandibular cast in occlusion while being mounted

Frontal View

For the mandibular portion, simply flip the articulator and repeat

Cleaning up all the mess and making the plaster nice and neat.

Keeping busy

Not only am I carrying a 26 credit hour workload, I am also doing research and currently the Vice President for the Student National Dental Association (SNDA), among other extracurricular activities. Needless to say, I am keeping super busy.
Last week my organization gave the Freshmen a big welcome and sort of an induction into the organization - we distributed some awesome t-shirts with these logos:

We also had our first general body meeting which consisted of a Panel of Dental Specialists who gave some background on their particular fields, passed down advice for those looking to specialize and answered all the burning questions the student body had on the topic. That was a success!

While all this was happening I had a Microbiology exam that I had to study for, a Pathology quiz, along with Restorative Operative and Removable Pros lab work. Fun! It's all about time management.

So cheers to a relaxing weekend! (well, sort of)

Man Dies From a Toothache...?!

This is really sad and just pinpoints the importance of access to health and dental care. This is similar to the death of Deamonte Driver, a young boy from Maryland who succumbed to a fatal brain infection back in 2007. This boy's family faced obstacles with medicaid, poverty and access to resources which resulted in an unfortunate and preventable death.Here, four years later we see that not much has changed. This young man, Kyle Willis died from a similar situation. He simply could not afford to purchase the needed medication.
This is just a reminder of the importance of health AND dental insurance and simply access to health and dental care.

Setting Teeth!

I love lab work! It's nice to get away from the books sometime.

I had such a hectic week. Restorative Operative exam was on Tuesday followed by Pathology on Wednesday and Microbiology on Thursday - all this while doing amalgam and composite preps and restorations AND making custom trays, record bases and wax rims. I will have a Restorative Operative graded exercise this afternoon which will include doing and amalgam prep and restoration on #12 and 31 and composite on 29.

I am happy this week is over - Cheers to the freakin' Weekend!!

National Board Dental Examination moves to Pass/Fail

It's official! According to the revisions to examination regulation by the Joint Commission on National Dental Examinations (JCNDE), National Board Dental Examination (NBDE) Parts I and II as well as the National Board Dental Hygiene Examination (NBDHE) will move to Pass/Fail effective January 1, 2012.

Why? Good news or bad? First of all the purpose of the NBDE is to assist State boards in determining qualifications of dentists who seek licensure to practice. The changes were made upon the recognition that 1. the scores were not being used for their intended purpose (ex. ranking dental schools, faculty evaluations, etc) 2. the scores are not necessarily a good measure of one’s academic ability 3. worry over it's integrity due to overexposure from re-testing and 4. some schools base acceptance to their residency programs heavily and many times solely on National Board scores – where they set a minimum score and everyone with scores below that minimum is basically “skimmed” off, not allowing all applicants a fair evaluation.

Research was conducted where residency program administrators were asked what they look for in an applicant and it was noted that the top 5 criteria were non-cognitive/non-academic, with the exception of Oral Maxillofacial Surgery programs which base their acceptance heavily on academic ability. With that said, the intention of the pass/fail system will allow the highlight of the other areas of the individuals’ abilities. But what does that mean for those who want to specialize? If there are no scores to compare candidates how do they select them? Will specialty programs implement a system where those seeking to specialize take another standardized exam? Those are questions that are still unanswered and quite frankly is not looking good for that group of people; but one thing is for certain, GPA, volunteer/extracurricular activities, Letter of recommendations and Personal Statements will be looked at with more scrutiny and carry a lot more weight than they currently do.

If the good intentions of the ADEA are met with those of the administrators of these
residency programs, I say this endeavor can be a good thing. Passing still stands at 75% and there will be no changes to the exam.

Also, effective April 2012 the endodontics diagnostic terminology adopted by the American Association of Endodontists will be incorporated in the NBDE Part II. I don't know what this is but, phew! all these changes.

What To Do With Your Summer Break-ish

RELAX!! need I say more? Unfortunately this will only temporarily help you get over the very hectic semester that just ended but may not necessarily do you any good in the long run if the time off is not used wisely.
Dental schools usually run right through the summer with the exception of a few weeks off which gets shorter and more hectic each year.

Some things you'll find us Dental Students doing during summer breaks are:

Research - Summer research is a good way to pursue your interests and learn something new outside of the classroom. They pay decent money, create mentor relationships, potential to publish, opportunities for (paid) travel to research meetings/symposiums and they look good on your CV
Study for the National Boards - Bah, Humbug! Most 2nd year dental students spend their summer studying for the boards. It's sort of like the rites of passage to clinic.
Externships - There are externship opportunities that may be affiliated with your school, or you can go to hospitals or clinics. It's a good way to continue to build on what you've learned in school and learn a few new things as well - step out and see how dentistry is in the real world.
Mission Trips - These can be fun! You get an opportunity to go out in the community and offer dental care. Mission trips can be both local (in the US) or international.
Work - $$ chi-ching! By this time in the year most of us are severely strapped for cash or have totally run out. Summer is a good time for temporary positions and a lot of places hire.
Vacation - Or we can simply opt to really get out and relax. If you have the money, this is a good way to get away, not think about anything school related for a while and come back refreshed.

Whatever you decide to do, use your time wisely!

Realistic Dental Robot (Manikin)!!

Wow! A life-like dental training robot that simulates the actions of an actual patient. This is incredible - a robot that blinks, moves it's tongue, moves it's head, chokes, coughs, sneezes, is incapable of keeping it's mouth open for too long and recognizes speech - like a real person.

Amazing!

After the check-up and work is finished on the manikin, it is not only capable of storing and analyzing each student's performance, but able to provide feedback through a computer link.
I must admit, though amazingly awesome, it is kind of creepy! I wonder when it will be available in the US...

I had a very busy and productive weekend! As Vice President of the Student National Dental Association (SNDA) - Howard University chapter for the 2011-2012 academic year, I had the opportunity to attend the 39th annual Convention in Baltimore, MD. The SNDA is an auxillary of the National Dental Association (NDA) and aims to promote and encourage the increase of minority enrollment and retention in dental schools.

I had a great time at the convention! It afforded great networking opportunities as I had the honor of meeting some very important people as well as fellow students from dental schools across America. We came together, shared ideas and experiences, formed new committees, etc. It was awesome! I was also able to stock up on instruments at very reduced rates, as well as snag some really cool freebies! and who doesn't like cheap and free?
Overall it was a totally enriching experience and I took away lots of information to share with my chapter.

Pedo: Stainless Steel Crown

The stainless steel crown/restoration is indicated for primary and young permanent teeth with multiple carious surfaces. It's also indicated for restoration of primary teeth after pulpotomy or pulpectomy procedures, class 2 lesions where the caries extend beyond the anatomic line angles, hereditary anomalies, etc.

After administering local anesthesia, placing rubber dam and removing caries, it's then time for crown preparation.

Occlusal reduction by 1-1.5mm using football diamond bur or 169 L tapered fissure bur
Proximal reduction using 169 L tapered fissure bur or thin tapered diamond bur
Buccal and Lingual reduction using same burs limited to occlusal 1/3rd

Select SS Crown, ensure proper fit, check margins for cervical adaptation extending 1mm subgingivally.

Crimp/contour the crown, cement it and voila!

Work, Play and Giving Back

Only a couple days left before the summer semester ends and us dental students get a break to have some fun, relax and revamp for the Fall. I couldn't be more excited for the break but I have finals to take care of before that can happen. Summer is a bad bad time for school; all the sun, sales, BBQs and pool parties are a huge distraction!

I Love Giving Back!

There was a community service event this morning that my school participated in (one of PLENTY). If you know anything about Howard University, you will know that we are big on giving back to the community and helping the disenfranchised. Here goes another distraction, for I absolutely love these events and try to make it to as many as possible. The look of gratitude on the faces that I am able to help and just knowing that I am making a difference and having an impact on someone's life makes me happy. Anyway, distraction: because I could or should have been studying for my finals since I haven't cracked a book all week but instead I was doing this:

The last graded exercise I had was to do a class II amalgam prep on tooth #18 - I passed. Since then I have been practicing class III and V preps which are, believe it or not, not as easy as they appear. The next graded exercise will be a Class III composite prep on tooth #8.

So I'm browsing the internet the other day catching up on the newest discoveries and trends in dentistry when I came across an article that made me laugh out loud, literally!

DENTAL TATTOOS???!!!!

My first reaction was: You have got to be kidding me!! According to orthodontist and implantologist Herman Verma, "the youth nowadays is quite adventurous and they take on new trends much easier than generations before them." Well, I don't need a doctor to point that out but a tattoo on your tooth? and check this, it costs between $250 and $500. Yeahh, miss me with that one. I'll gladly place them on though ;)

Since I'm Up Studying...

First lecture exam of the summer in a couple hours and can I tell you, I have no motivation to study. It's summer daggone-it! Can I get a break around here?
The exam will be on Class I,II,III,IV and V amalgam preparations, Class III, IV and V composite preparations and liners & bases. It's pretty much a test to see if we understand the theory behind all the work we've been doing in the Sim lab: retention form, resistance form, convenience form, convergence, divergence, pulpal floor depth, cavosurface line angles, axio-pulpal line angles, etc.

Here are examples of what I'm talking about:

Class I (OL)

Involves Pits and fissures of posterior teeth, like the one pictured.
Also involves lingual pits of maxillary anterior teeth and Occlusal 2/3rd ( Buccal/Lingual) of Posterior teeth.

Class II (MO)

Involves Proximal surfaces of Posterior teeth

Class III

Involves proximal surfaces of incisors and cuspids, does not involve the incisal edge

Class IV
Like Class III but it involves the Incisal edge. Look closely, u can
see where the restoration differs from the actual tooth

Class V

Exam in the morning! I have to do an MO (mesio-occlusal) amalgam cavity prep. on tooth #18. A tad bit nervous about it because I really only did it once in class. I'll try my best to do well though and I'll be sure to post the photo after it has been graded.
Speaking of photo, I need a new camera. I have this Sony Cybershot DSC-T90 which I looove, it's the cutest thing but close up pics never come out right. I'm thinking along the lines of a Nikon D3100 -that would totally make my year! but it's $700, which is not in my budget at all. It would be cool to have clearer pictures to share though, especially when I start filling the cavities and making prostheses.

After this exam tomorrow, our next project(s) will be preparing Class III, IV and Vs. Those should be fun. I was perusing blogs the other day and came across this photo on Dental Dad Diary's blog. How cute are those little Class V men?

Remember as a kid when we were taught about Thomas Edison, Philo Farnsworth, Percy LeBaron and the likes? We swore we would one day be like them and invent something really cool! Our little brains would whir into action, imagination running wild as we collected any trash, build some contraption and give it function/life. That enthusiasm soon died though as we grew older and realized, sigh! all my great ideas are already taken.

Then I happened on this little photo. As simple as it is, it sparked that enthusiasm again. Maybe inventing a dental instrument is in my future. I feel the gears in my brain turning.

Occluso-Lingual Class I

I love it when I set a goal for myself and it is reached. I get all kinds of happy and it motivates me to keep moving forward.

As you may re-call, my graded exercise for an Occluso-Lingual amalgam cavity prep on tooth #3 (maxillary right first molar) was on Wednesday. I did really well on it and so I'm one happy camper!

Occlusal Class I

Remember that Class I Occlusal amalgam cavity prep./graded exercise on tooth #30 that i told you about in the last post? (pictured below).

This is how you prepare the tooth for the amalgam restoration

330 bur used to make initial punch cuts in the occlusal fossae
Proper outline established
Mesial and distal dovetails made and divergent. Slight buccal and Lingual extensions established
Depth of 2mm established
Pulpal floor made smooth
Done! in under 30 mins using just the 330 bur

I did pretty well on it and now I can move on to the next project on hand, a Class I Occluso-Lingual amalgam cavity prep. on tooth #3. yes, indirect, like this (below)

The graded exercise for that will be on Wednesday and I'll be practicing for the next couple days in hopes of getting that perfect score. Picture to come after it is graded.

I'm still high off memorial day weekend. It was too much fun! I had a blast with the family and close friends in no place other than the BIG APPLE! But all things good must come to an end...

Had my first graded exercise at the top of the hour this morning. We had to make a cavity prep. for amalgam on tooth #30. Let's hope I did well and that my weekend didn't have my hand-eye coordination out of whack.

Every semester sucks while your in it until it's all over and you think, hmm that wasn't so bad afterall.
There were so many classes that I didn't care for this past semester, like Histology. I wish to never see another Histo slide for as long as I live, it was simply tortuous. Physiology wasn't bad. It started off great, took a nose dive around mid-semester and came right back up flying - talk about a roller coaster ride. Dental Materials was all a matter of knowing the different materials used in Dentistry. You would think this would've been an easy class but it was on my most feared list because of all the things I heard about it from upperclassmen. However, it turned out to be a-okay, though it was A LOT of studying for the amount of credits. My other four classes... big yawn. There were 2 exams for the entire semester which means no room to "mess up" but studying for those courses beat studying for Histo anyday.
At the end of the day even though I disliked this semester to the core I did pretty well in my classes and that's all that matters, next!

This has been one long and tortuous semester. It just will not end!

I find this picture appropriately fitting at this particularly moment as I waste away another perfectly good weekend studying. At this point in the semester I'm dwindling and it's taking extra extra umph to get over the finish line; BUT only three more finals to go until it's hasta la vista D1year! Bitter-sweet I tell you - Sweet because I'll officially be a whole year closer to my goal & bitter because up means double the toil and trouble.

Let's hope there are no witches waiting for me on the other side with their fires a burn and cauldrons a bubble *shudders (figuratively speaking, of course).

Battle Scars and All

There was a period when I didn't mind being asked this question. It could be a friendly gesture, a conversation starter or being genuinely concerned. But, after a while it gets old...and irritating. What do you mean if I'm tired? I'm a Dental student! Oh, and it's worse when your own classmates ask. Last time I checked buddy, you and I had the same course load. Why aren't YOU tired?!

One another note, it's finals period. That period when we all burn the mid night oil in hopes of meeting our semester's goals of all A's, being every professor's favorite, break a class record, climb to the top of the Dean's list and graduate summa cum laude - wait, that's undergrad. Nevermind.

Fast forward to present moment: 6:44 am, sleep deprived, crouched over mounds of books and study sheets while the computer silently protests about being over-worked. I'm only 76 hrs 14mins and 9 seconds away from what's about to be one of the toughest weeks in the entire history of my dental school career; all 10 months, 10 lbs, 4 grey hairs and $62,000 of it. Next week is gonna be muddy! Like Yokozuna versus Verne Troyer in a sumo ring, muddy. But I have full confidence that the little guy will prevail and come out victorious, battle scars and all.

After the next 2 weeks I'll be 1/7th a dentist!! (including residency). Baby steps people, baby steps.

DDS Flow

The next three weeks are about to be ca-ra-zy! It's almost the end of the semester which translates to grind time. A couple of my classmates like to refer to this as the #ddsflow - like, "man! it's so nice out whyyy do I have to be stuck in the library?! Oh Well #ddsflow," or " whoa! did u see our final exam schedule?! It's about to go down! Serious #ddsflow." I guess we just figured it's a nifty way of saying we have to study without feeling too stressed about it while still keeping our goal in check. It works.
So, we pretty much have one more regular exam in Physiology (Renal) before we dive neck-deep into the gazillion amount of info accumulated for our 7 classes...7!

I despise amalgam! Not the procedure of actually placing the amalgam but the material itself. It just doesn't work well with me - not a team player at all.

So...get the amalgam tablet, place it in the amalgamator, use amalgam carrier to place it in the prep., compress amalgam with condenser, use cleoid discoid to make anatomy. Straight forward. Except, I ended up doing this procedure all of 4 times before finally getting it right. First I had no contact, Second the amalgam was under triturated, third I had no anatomy Sighh!

Anyhoooo, matrix band in place, amalgam seated. This was my FOURTH trial.

Go ahead and ignore that awkwardly rotated central incisor

And Alas! a Final Product that my professor was pleased to sign off on. Phew!

Pre-Sprng Break aka Mid-Term Exams had me MIA for a while. Now that it IS Spring Break I have a little time on my hands and what would I rather talk about but the obvious...Spring Break.

Enjoy this re-visited post, it really doesn't change:

As Dental Students, we spend most of our time in the classroom, in the lab, or in the clinic and when we're out, we're studying - the cycle seems almost endless...until it's Spring Break.
Spring Break for many is as a time to unwind, a time to relax, go out and have fun, even travel! Spring Break is all about relaxation, yes? No, not necessarily.
Many people make the mistake of getting lost in this carefree period. Yes, it's a time to unwind, but it should also be used as a time to prepare yourself for the last leg of the semester. If you find that the first half of the semester didn't go so well, maybe Spring Break is a good time to catch up or it may be a time to get ahead. All these are good whilst taking a breather from the classroom routine.

So before you fly off to Cancun to enjoy some fun in the sun, remember spring break, while it should be used to relax, is also an important time to revamp, catch up and to get ahead to make a difference in the final leg of the semester.

Make those grades! and enjoy...

Dating a Medical/Dental Student Fact Sheet

Saw this on FOX News and thought it was hilariously true. I changed it up a bit to incorporate the Dental Student in it.

Dating a med/dent student? Check out these tips for a "healthy" relationship.

1. Don't expect to see them. Ever.

2. Accept the fact they will have many affairs. With their books.

3. Learn to hide your “ew, gross” reactions when they tell you all the stuff you never wanted to know about your bodily functions or what they encountered in the clinic today.

4. Support them when they come home after each test, upset because they failed—and gently remind them after they get their well above passing grade how unnecessary the “I’m going to fail out of medical/dental school and never become an MD/DDS” dramatics are.

5. Each week they will have a new illness. Some will be extremely rare, others will be more mundane. Doesn’t matter. They will be certain they have it (no second opinions necessary.) Med/Dent school can, and will, turn even the sanest into a hypochondriac. Date them for long enough, and you’ll become one too.

6. There will be weeks you'll forget you even have a significant other—friends will ask how he/she is and you'll say, “What? Who? Oh....right. He's well...I think.”

7. They'll make you hyper-aware that germs are everywhere and on everything. Even though you used to walk into your home with your shoes on, and sit on your bed in the same clothes you just wore while riding the subway, or sat on a public bench in, you'll become far too disgusted to ever do it again. Believe me, it's going to get bad...you'll watch yourself transform into the anal retentive person you swore you'd never become. And when you witness others perform these same acts that, before you began dating your med student, you spent your entire life doing too, you'll wince and wonder, “Ew! How can they do that? Don't they know how many germs and bacteria they're spreading??!”

8. Romantic date = Chinese take-out in front of the TV on their 10 minute study break.

9. A vacation together consists of a trip down the street to Walgreens for new highlighters and printer paper.

10. Their study habits will make you feel like a complete slacker. For them, hitting the books 8-to-10 hours a day is not uncommon, nor difficult. You'll wonder how you ever managed to pass school on your meager one hour of studying per night.

11. They're expected to know everything. Everything! The name of the 8 billion-lettered, German sounding cell that lives in the depths of your inner ear, the technical term for the “no one's ever heard of this disease” disease that exists only on one foot of the Southern tip of the African continent. But ask them if your knee is swollen, or what you should do to tame your mucous-filled cough, or why the heck your head feels like someone's been drilling through it for oil for two weeks straight, and they won't have a clue.

12. “My brain's filled with so much information, I can't be expected to remember THAT!" will be the standard excuse for forgetting anniversaries, birthdays, and, if you get this far, probably the birth of your first-born.

13. If they DO happen to wish you happy birthday (even when it's not) consider it and celebrate...pronto. Despite dental school conditioning of their organizational skills, their schedules don't have space for anniversaries and b'days. Understand?

14. When they tell you that dental school should have a sitcom of their own on the lines of Scrubs, House, Grey's Anatomy, et al...Just nod

15. You'll need friends with unending patience who pretend never to get sick of listening to your endless venting and complaints. Or, you'll need to pay a therapist who will pretend never to get sick of listening to your endless venting and complaints.

But take this all with a grain of salt. It's not like I'm speaking from experience or anything.

If you know me or have been reading my posts long enough, you know I'm not a fan of the winter. I'm literally freezing at any temperature below 74 degrees, get to 30 degrees and you can just go ahead and label me brain dead. But that was NYC winter. See, DC winter is a lot different.
In NYC there has to be at least 1 1/2 ft of snow for there to be a designated snow day. Unlike DC, NYC knows what a snow plough is, and that it's used to move snow so that people can go on their merry way and carry on with their lives. In DC, it only takes ONE INCH of snow (no joke) before all hell breaks loose. I'm talking about fallen electrical lines, no electricity, no heat, no hot water, fallen trees, motor vehicle accidents, traffic for days and... school delay <--There is a God
For God knows that sometimes we need that extra 2 hrs either to catch up on some sleep or to catch up on studies that the previous day did not humanly allow. I've actually come to kind of sort of like the winter now, except the whole freezing, below 30 degree, long johns, coat, scarf, glove wearing part. Just send on the sleet and snow and we have ourselves a deal. Seriously, as the Gods will have it, they know that sometimes we need that break because of our grueling daily schedule from 8:00am on most days to 5:00pm. At the end of the day, most, if not all of us are toast. It is very difficult to open up a text book or power-point slide right away after spending approximately 8 hours in the classroom on a daily basis. So, when the opportunity arises for a 'mini snow-break,' it's welcomed with opened arms. It serves as an opportunity to rejuvenate. Strangely, but true, a school delay on a snow day makes a big difference between an 8 hour grueling day and a 6 hour day. I prefer the 6 hour days (of course), at the end of it I feel more energized and motivated to work.

Now, how do I get all this snow/ice off my car?... Ugh! I hate snow >:-[

I'm thinking about what specialty of Dentistry I should go into, or if I should specialize at all.

For a very long time I have been pretty hooked on Orthodontics (study and treatment of malocclusion aka placement of braces) but recently Oral & Maxillofacial Surgery has peaked my interest. Maybe it's all that Anatomy and dissecting cadavers that I did last semester. Either way it's a very interesting and versatile field where you're constantly tested and it seems like you'd never get bored. Might I add that it's muy muy lucrative! I mean, after 4 yrs of Dental School and 4-6yrs thereafter in residency, I'd say it better be!

This is how Dental School is sometimes:

knocks the air out of you

It's the end of a very stressful week but just a few hours shy of another. I must have started writing a post about three times this week. Let's just say other much more important duties called.
This past week left me on an emotional high. My week began with taking 2 examinations and the game plan to use the rest of the week to study for 2 huge ones that I have next week (Histology and Physiology). A day after taking the 2 exams the results for one came out and, I was not pleased. I tend to set really high standards for myself and it takes a toll on me when the standard isn't met. At the same time, my class received an email about retaking the Dental Materials examination that we had last Friday because the class"supposedly" did not perform well. I was pissed, to say the least. For the moment I was mortified because my game plan approach for preparing for next week's examinations were thrown off.
So mid-week, after catching up with all my Histology lectures, I was able to make the mental adjustment and temporarily shift my mental resources and review for Dental Materials, which by the way is probably just as voluminous as Histo. and Physio. Now, the weekend is upon me and my grind for next week continues. Can I catch a break around here?
But, although I was upset about not getting a particular grade on an exam, the re-take, and though my game plan was thrown off, I learned a valuable lesson: Things aren't always going to go as planned but you must have the mental fortitude to suck it up and press on, or as I've found myself saying quite often these days - just roll with the punches.

It has begun

This week went by too fast! Perhaps that's the effect of an 8-5 schedule ( I like to call it my full-time job) coupled with many hours of studying for examinations which, by the way, have now begun to snowball - oh joy! (insert sarcasm). Either way I'm happy to announce that I am thoroughly enjoying Dental School - well maybe not thoroughly...there is this one class that is really grinding my geers (mutters indecencies under my breath) but it's great for the most part.
It's the weekend! yay? not really, mine constitutes studying for two examinations which are on Monday (groans) but at least I don't have to wake up at 6am. Funny how that becomes the highlight of my weekend.

Follow through or play catch-up

Every semester comes with new challenges. Hey, this is nothing new, but still I must remind myself. I'm now in second semester of my D1 year and taking seven courses. While this is not my first attempt at taking seven courses, i am forever reminded of the challenges that comes with this if I am not proactive in my actions. I remember the last time I took 7 classes. it was a struggle because I allowed myself to play catch-up in at least 3 of those courses because in the beginning of the semester I took them for granted.Though an arduous task, I did managed to complete all of my course work with B's or better. It wasn't easy as I had to dig myself out of the ditch I put myself in by performing in a sub-par manner on my first examinations. I did not take full ownership of what I was suppose to do from the beginning. Fast-forwarding some two years later, here I am taking 7 courses again. The mistakes of the past I now use as a spring board for my current studies. Now, I am taking nothing for granted because I realize that if I don't apply myself as I should, I will forever be playing catch-up.
History has a way of repeating itself ! yet, I've come to realize that this can only happen if I allow it to, which I do not intend on doing. My words to myself are to follow through and be proactive, rather than fall through and play catch-up. Food for thought!

Reducing Fluoride Levels In Drinking Water?

The U.S has come very far in incorporating fluoride into the drinking water supplies making it part of the daily nutritional intake. This process has been credited for the decrease in the occurrence of tooth decay. On January 7, 2011 however an announcement was made by officials from the Department of Health and Human Services (HHS) and the Environmental Protection Agency (EPA) lowering the recommended intake of fluoride in water to 0.7 milligrams per liter, less than the current range of 0.7-1.2 mg/L. Though it is said that the water fluoridation process has decreased the occurrence of tooth decay, it has also increased the occurrence of dental fluorosis in young children - a condition where enamel forming cells are damaged causing the tooth's enamel to become porous resulting in a mottled, white/brown appearance.

The proposed recommended intake is exactly what it is, a proposition. More research will have to be done to find out just how much fluoride we actually are supposed to be consuming via drinking water.
Am I the only one that thought this issue was totally resolved by the dental and scientific community?

Perspective: first semester transition to second (D1)

In conversation with a former professor of mine he reminded me that "continuity enhances growth." As I mull on that statement, I think of the upcoming semester (S2-D1). One thing that struck me is that having the right mental mindset in anticipation of the course load ahead is a major attribute as I transition from semester 1- D1 to semester 2-D1. Simply put, going from 4 courses in S1-D1 to 7 courses in S2-D1 warrants an acknowledgment of a working transitional plan, coupled with adopting a prize fighter, readiness attitude. After all, this will help solidify the statement made by my professor.

Hopefully everyone has formulated their transitional plan for this semester. If not, you better get on it!

ENOUGH!

Christmas has come and gone, New Year's is a day away and I feel like I'm on a year long vacation.

I know I'll get nasty stares for this, maybe a couple neck snaps and a few "uh-Uuhhh's" but...

I miss school!

I really only needed a week off, just to catch up on some sleep, have a little fun, spend time with the family and to be a little carefree. The only other thing that would keep me from not thinking about school would be to have gone to a foreign country with lots of sun and white sand beaches

HEAVENLY!

...which I clearly didn't do since I was up to my knees in snow the other day. I pretty much have a love-hate relationship with NYC. More love than hate though.

So I'm spending another relaxing day at home today (until it's time to ring in the New Year, of course) and can't help but think of all the stuff I'd rather be complaining..err learning about this semester. I've just about had enough of this break. I've had enough of the carefree lifestyle for now. I'm ready to get back to business!

Santa Baby I've really been an awful good girl this year and I've got good grades to prove it. I finally got the final results from my classes and I couldn't be more proud! It's Beginning to Cost A lot like Christmas

It's that time of year again when chestnuts roast on the open fire, Jack Frost nips at your nose, Yuletide carols are sung by a choir and folks dress up like Eskimos. Yes...everybody knows 'tis the season for family, close friends, snow and lots of spending!

Since I've become a broke Dental Student my Christmas Wishlist or every Wishlist for that matter just seems to be growing longer and longer. Did you know that during the Christmas season the average person anticipates spending $556 in gifts alone?! and I thought the Country was still in a quasi-recession.

Anyhow, I have a dilemma. I have to buy gifts for the family and friends but have a very limited budget. Can you help me out? Using the numbers 5,5,5,1, and the four basic arithmetic operations, see if you can derive the number 24. See, $24 is the amount of money I'll be spending on gifts for each member of my household. Hey! don't judge me it's the thought that counts :)

[Deep sigh of sweet relief] I've successfully completed my first semester of Dental School *throws confetti*

Dental School has definitely been a new and unique experience. Academically, it is challenging but doable, and with some discipline and tenacity I was able to get through it unscathed. What I've learned: Start strong and keep up! If you allow yourself to slip, you will forever be playing catch up.

As anticipated, dealing with different personalities was also a challenge; however, like everything else, deal with it as it comes and adjust accordingly and if needs be. What I've learned: You can't please everyone and at the end of the day you can only be you.

I'm now on Christmas break and I have every intention to make it as fun and relaxing as I possibly can albeit the freezing temperatures. Get ready for my holiday randomness!

My First Set of Wax-Ups

Just a couple more days before the end of my first semester in Dental School and I must say Occlusion has been one of the more fun, stress free classes. If you recall I had my "Initiation" into Dentistry through this class when I carved my first tooth out of wax. As the semester went on and we learned more and more about each tooth's anatomy and intricate details we had to do a little bit more than just carve teeth. We had to do what is called a wax-up/wax drop.

Waxing up teeth involves wax, a colored marking pencil, a tooth peg, dentoform/typodont, bunsen burner, specialized instruments, knowledge of dental anatomy, a good eye for detail and nylon :) for the sparkle. The actual waxing skills are developed on the way.

Essentials

Waxing up is in essence building a tooth from wax. Now this may sound easy but keep in mind that this has to look like an actual tooth and actually be functional, i.e the aim is to reproducibly relate the maxillary (upper) and mandubular (lower) teeth; they must contact each other (not too long, short, thick or thin). This ordeal includes placing and removing wax, making marginal ridges, sealing margins, shaping the tooth, establishing occlusal contacts, and evaluating the final wax-up.

I can see the finish line

From classroom to labs to studying and back again...seems like a never ending cycle until you realize it's almost the end of the semester. Christmas is approaching and I've never been more excited to see it come! More important at this time though are finals.
Only two more weeks remain in the first semester of my D1 year as finals approach. Thankfully at this point I am in good standing with all my classes and need not fret whether I'll pass a class or not.
For the next two weeks I'll be down under preparing to turn the B's into A's and finish off the first leg of my race correct, and with no regrets.I do look forward to the end of this semester and the beginning of the next. I guess one year from now I'll be saying the same thing until my final semester as a D4; until then, I'll continue to keep my eyes on the finish line.
